Laparoscopic Treatment for Inguinal Hernia with Adult Cryptorchidism

Abstract
Purpose: Approximately 7 percent of children with inguinal hernias also have cryptorchidism. However, adults rarely have this combination. We evaluated the feasibility of conducting laparoscopic transabdominal preperitoneal (TAPP) herniorrhaphy combined with orchiectomy.

Methods: From September 2012-December 2016, we treated 17 men diagnosed with inguinal hernia and cryptorchidism. TAPP herniorrhaphy and orchiectomy were conducted in these patients, and operation time, complications, recurrence, and chronic pain were evaluated.

Results: Of 846 men with inguinal hernias, 17 also had cryptorchidism (2.0 percent). Inguinal hernia was on the right side in 10 patients (58.8 percent), on the left side in 3 (17.6 percent), and bilateral in 4 (23.5 percent). All cases of cryptorchidism were unilateral, with 11 patients (64.7 percent) having it on the right and 6 (35.3 percent) on the left side, and all were accompanied by inguinal hernias on the same side. Average operation time was 30.3¡¾9.9 min, and average length of stay was 13.7¡¾8.1 hours. There was no post-operative recurrence and chronic pain. Mean follow-up was 29 months.

Conclusion: TAPP with orchiectomy is feasible and safe for treating adults with inguinal hernia and cryptorchidism.
